At Sagent, our adult rehabilitative mental health services (ARMHS) are designed to meet you exactly where you are. We provide the support and initiative needed to help you reclaim your independence, build lasting resilience, and move upward on your journey toward a fulfilling life.

What Is ARMHS? A Partnership for Real-World Success

ARMHS is a community-based service for adults (18+) who are managing a mental health diagnosis. Unlike traditional therapy that happens in an office, our mental health practitioners work with you in your home or out in the community to practice the skills of daily living to support your autonomy.

Our Integrated Approach to Skill-Building

  • Restoring Daily Living Skills: From managing your household and budget to navigating public transportation, we offer supportive tools to help you master the logistics of a stable life.
  • Social & Emotional Resilience: Stronger social connections can help you navigate daily life with more stability. You’ll learn to advocate for your needs, manage symptoms in real-time, and utilize community resources with confidence.
  • Crisis & Relapse Prevention: We help you identify triggers early and develop practical skills to manage them so you can maintain stability during challenging moments.
  • Transition Support: If you are moving from a residential setting back into the community, we act as your bridge to ensure the transition is smooth, safe, and empowering.

Who We Serve: Is ARMHS Right for You?

Everyone is worthy of a life that feels manageable and hopeful. To be eligible for ARMHS at Sagent, you generally must:

  1. Be 18 years of age or older.
  2. Have a mental health diagnosis (such as depression, bipolar disorder, schizophrenia, or PTSD).
  3. Have medical assistance (MA) or a qualifying PMAP (Prepaid Medical Assistance Plan).
  4. Experience a functional impairment in one or more areas of life where skill-building would help you regain independence.

ARMHS is highly inclusive, so our practitioners meet you in your home, at a local library, or out in the community—wherever the real-world practice is most beneficial for your goals.

Think of therapy as the place where you process the “why,” and ARMHS as the place where you practice the “how.” While your therapist helps you understand your emotions, your ARMHS practitioner helps you apply those insights to daily life, like going to the grocery store, talking to your landlord, or organizing your schedule.

We believe in a flexible approach. Depending on your needs, you may meet with your practitioner once or several times a week. We collaborate with you to create a schedule that fosters progress without feeling overwhelming.

Absolutely. In fact, we encourage it. Our ARMHS practitioners work in harmony with your Sagent therapists and psychiatrists to ensure your support network is seamless and your care is integrated across all fronts.

Starting your journey is simple. You can request an assessment directly through our website or have your current provider send a referral. We will meet with you to determine eligibility and begin mapping out your path to independence.
